Once assigned to an approved clinical placement, please complete the following requirements prior to the start of your clinical experience.

Legacy Health Requirements:

All students and faculty must complete these education modules annually prior to a clinical placement at a Legacy Health clinical site.

  1. Students/ Faulty: Prior to first clinical experience, read Legacy policies Clinical Experience Policies

  2. Students / Faculty: ReadWelcome to Legacy Student and Staff Expectations PowerPoint presentation.

  3. Students / Faculty: Sign the Legacy Health Nondisclosure Agreement. Witness signature should be clinical instructor or school designee. Update as a post-assignment clinical requirement in ACEMAPP.

  4. Glucose Meter Training:
    Cohort & Precepted Students: Students no longer have an individual ID for glucose testing. Patient blood glucose tests and glucose meter quality checks must be completed with a Legacy-employed nurse (married state). Legacy nurse will be responsible for training, observing, and testing using their Legacy employee ID number.
    Testing must be completed in the “married state” under direct supervision of the Legacy nurse. NO exceptions allowed. Two comments need to be added to every patient test completed by a student: “RN notified” and “Student nurse testing”. Upload completed StatStrip Training Certificate as post-assignment clinical requirement in ACEMAPP, “LHS StatStrip Completion Certificate”.

  5. Students / Faculty: Complete Epic Training. MUST be completed before you begin your clinical placement. Contact your school's student placement coordinator to receive your login ID and temporary password. All password changes must first be made at mypassword.lhs.org. Once your password has been changed successfully, you will be able to sign into Legacy’s E+ Learning and Performance Management System so that you can complete the assigned self-learning modules. Modules assigned to all students/faculty are:  EPIC Nursing Student  Non-employee Mandatory Education  POC Nova StatStrip 1.75 Glucose Meter Initial Training and Competency  MRI Safety Issues  Unity Center for Behavioral Health ONLY: UCBH Badge Access All self- learning modules MUST be completed before you begin your clinical rotation. Once completed, upload completed E+ training certificate to ACEMAPP under the Other Requirement #3 tab. Epic Training needs to be completed one time only, student/ faculty do not need to repeat for future clinical rotations with Legacy.

  6. Omnicell MedStation Registraion. Login ID for Omnicell is the same as assigned for Epic. Initial Omnicell password will be the same password that user has set-up for Epic access

  7. Students / Faculty: Complete the Legacy ID Badge Request Form. Form must be signed by authorized school personnel. Take to any Legacy security office to receive badge. No more than two students at a time should be waiting to obtain their badge. An entire cohort should NOT be sent to security at the same time.

  8. Students / Faculty: Read Legacy Care Transfer Communication Process

  9. School Placement Coordinator: Complete the Student Access Spreadsheet for students and faculty at least 30 days prior to start of clinical rotation. Email to Deb Henry and Legacy Security. Email address for both are included on the document.

  10. Students / Faculty: Read the Married State Preceptorship Model.

  11. Students/ Faculty Assigned to Unity Center for Behavioral Health complete the following:
    Unity Badge Access Training
    Unity Badge Access Training will be assigned in Legacy’s E+ Learning and Performance Management System. This self-learning module must be completed prior to first clinical day at Unity.

  12. Students / Faculty: Assigned to Legacy Randall Children's Hospital: Read Expectations for Nursing Students.

  13. Campus Site Maps: Emanuel/Randall Children's Hospital, Good Samaritan, Meridian Park, Mt. Hood, Salmon Creek
