Form Hours Report

Form Hours Report

The Form Hours report allows you as the School Coordinator/Instructor to view the total hours that students have logged for a specific course. This report will also provide further details including what forms were approved by the school and the specific rotation ID number. You may view the results in your browser or export this data to a spreadsheet for your records.

Step 1:

From your home screen, select "Reporting" from your main menu options.

Step 2:

Select the "Forms Report" in the list of available reports.

Step 3:

Complete the required information, including the course and course status:

  • Complete: This status means that the member has already completed the course.
  • Active: This status means that the member is currently assigned the course and the course is active.
  • All: This will include both Completed and Active courses.

You can further narrow down your search by including a date for courses either "Starting After" or "Ending Before" a specified date.

Step 4:

Click the "Submit" button to view the results.

NOTE: You can also click the "Generate CSV" button to export the results into a spreadsheet. Once the CSV has been created and is ready for download, the button will change to "Download CSV". Click this button to download the file. If you are looking to determine the sum of all course hours, this step is necessary.

Once a student completes a Form required for his/her course and/or rotation, you may have the ability to approve or deny this document. If you have the ability to review this document, you will have a notification on your ACEMAPP homepage. If you do not have a notification, but believe you should be completing this, please contact your school.
